Been MIA Looking to Rejoin

I've been MIA for quite awhile due to work and starting a family. Life has settled down and I'm really interested in rejoining the clan. This is the first time I've gone MIA so I'm unsure if I need to reapply or what the procedure is.

I submitted a new application, but I'm not 100% sure it went through. I never received a confirmation it was transmitted.

Is there a different or updated application link? The link the in [OBC]Membership thread doesn't seem to work. I fill it out completely, click submit, and it just resets all the fields.
